Please note the following instructions before filling and submitting the below application form. Please ensure that you have all the information ready with you before you submit the form. Also please submit the form only if you are certain that you will be sitting for the exam.

NOTE: SUBMISSION OF THE APPLICATION FORM DOES NOT GUARANTEE A SEAT IN THE EXAM. THE SEAT WILL ONLY BE CONFIRMED ONCE YOU COMPLETE THE PAYMENT OF EXAM FEES AND OTHER PREREQUISITE DOCUMENTS.

  1. Enter all the relevant data in this form (ensure all the entered data is accurate).
  2. Thereafter, submit the form.
  3. Online Fees Payment: We will send you an email with the online payment details (online bank transfer / bank deposit) for the Exam fees.
  4. Once the payment is done, please email us the exam payment receipt (screenshot / photo) along with photos of the prerequisite documents for verification.
  5. Please also attach a photo / scan of your NIC or Passport (which ever you used to register for the exam).
  6. Please ensure that the NIC / Passport Number, your photo and your name are all clearly visible.
  7. Once we have verified the payment and prerequisite documents, we will confirm your seat, and you will receive the payment receipt and the exam details by email.
